Initial reports say the man, identified as Fernando Salazar, 61, was driving a dump truck along Highway 16 near the intersection of Highway 221 at 10 a.m. on Saturday morning when the accident occurred. Witnesses reported seeing the truck fishtail before Salazar lost control of the vehicle completely. The truck rolled several times, and Salazar was partially ejected.
