Fatality at Amazon Warehouse
Last Sunday, 52-year-old Jody Rhoads was involved in a fatal accident at the Amazon warehouse in Carlisle, Pennsylvania on Sunday.
About the Fatal Accident at Amazon
According to ABC, Rhoads was killed when the motorized pallet jack she was operating crashed into palette shelving and she was pinned. She was later pronounced dead at the Carlisle Regional Medical Center as a result of multiple blunt force injuries. She leaves behind two sons, three brothers, and four grandchildren.
Amazon is working with the Occupational Safety and Health Administration in its investigation, which could take up to six months. OSHA will determine if any safety standards were violated by Amazon. If there are violations discovered, that could lead to the issuance of citations.

The following facts were taken from the Bureau of Labor Statistics:
- There were 12 worker fatalities in warehouses during 2012.
- The rate of injury and illness was 5.5 for every 100 full-time workers in 2012.
Most cases involved either days away from work, job restriction, or transfer at a rate of 3.9 per 100 full-time workers.
