A man was killed after he was crushed by mushroom pallets at work on Monday evening.

Details about the Fatal On the Job Accident

The Orlando Sentinel is reporting that a man from Lake County was crushed to death by pallets of mushrooms on Monday evening, according to the Orange County Sheriff’s Office.

The man, who has been identified as 45-year-old Bruce Edward Stephenson Jr., of Tavares, was reportedly moving pallets of mushrooms with a forklift at Monterey Mushrooms, Incorporated on Sadler Road at about 10:25 p.m. when the pallets began to shift. Stephenson attempted to jump off the forklift, but the pallets fell on him before he was able to get away. Stephenson was transported to an area hospital he was pronounced dead.

Workers who witnessed the accident were “very concerned.” The Occupational Safety and Health Administration issued Monterey Mushrooms a $6,000 penalty in December for a serious violation of exposing employees to a fall hazard in the harvest area.
